Confined Space Entry Permit

Purpose. The crew leader completes this permit before any person enters a confined space and closes it when everyone is out.
1.When this permit is required
This permit is required for every entry into a space listed in the confined space register and for any other space that meets the definition in the Confined Space Entry Procedure. One permit covers one space for one shift. A new permit is required if the crew, the work or the conditions change.
2.Completing the permit
The permit holder must be the crew leader on site. Record the space, the work, the hazards identified, the isolation of flows and energy, the ventilation, the gas test readings before entry, the entrants and the standby person, the rescue plan and the communication method. Every entrant and the standby person must sign on.
3.During the entry
Continuous gas monitoring readings must be recorded at least every 30 minutes. If the standby person changes, the new standby person must sign on before the previous one leaves the entry point.
4.Closing the permit
The permit holder must confirm every person and item of equipment is out, the cover is replaced and the isolation is removed, then record the finish time and sign off. Enter the permit in the Permit to Work Register with status Closed.
