Plant Shift Handover Report Template

Purpose. Plant operators use this template to hand over the state of the plant, critical control points, chemicals, alarms and outstanding work to the incoming operator at every shift change.
1.Why handover matters
Most process upsets happen in the hour after a shift change. A complete written handover, read and signed by both operators, is the control that prevents an alert level or a dosing change being lost between shifts. Handover takes at least 15 minutes and is done at the control desk with the supervisory screens open.
2.Process status
Record the raw water flow, turbidity and pH, the coagulant and polymer doses, each filter's run time and headloss, filtered water turbidity, contact tank residual and contact time, fluoride concentration, clear water storage level and any process that is running outside its normal range.
3.Critical control points
State each critical control point's current value against its target, alert level and critical limit from the Critical Control Points Register. Record any excursion during the shift, the action taken and whether the control centre was notified.
4.Chemicals and deliveries
Record bulk tank and day tank levels for hypochlorite, alum, polymer, fluoride and lime, any delivery received with the docket number and strength, and any dosing equipment fault.
5.Alarms, work and safety
List alarms that occurred and their status, contractors and visitors on site, work orders open, permits in force, and any hazard or incident. State what the incoming operator must do first.
6.Sign-off
Both operators sign the handover. The outgoing operator does not leave until the incoming operator has read and signed. The report is filed in the shift log and reviewed weekly by the Water Treatment Manager.
