Water Quality Incident Report Template

Purpose. The incident controller uses this template to document a water quality incident for the debrief, the regulator and the risk management plan review.
1.Purpose of the report
This report is the formal record of a water quality incident. It is completed by the incident controller within ten working days of close-out, presented at the debrief, sent to the health regulator where notification was made and used to update the risk management plan. Write it so that a reader who was not involved can understand what happened and why.
2.Event summary
State the date, time and location of first detection, how it was detected, the incident level, the population and assets affected and the date and time the incident was closed.
3.Timeline
List every significant action with its time: detection, notifications, isolations, sampling, results, customer advice, corrective actions and clearance. Use the incident register and control centre log as the source.
4.Cause and contributing factors
Describe the hazard and the hazardous event, which barriers failed or were absent, and what conditions contributed. Refer to the critical control point records and the risk management plan.
5.Actions and lessons
List corrective and preventive actions with owners and dates, changes needed to procedures or the risk management plan, and any training required. State what worked well.
6.Approval
The report is reviewed by the Control Centre and Water Quality Manager and approved by the Managing Director before it is sent to the regulator.
